HOW DOES WEIGHT LOSS MEDICATION WORK?

There are new weight loss medications now available to support weight loss.

They work by mimicking a natural fullness hormone called GLP-1, which is produced by the gut after eating. These hormones signal to the brain that you are full, and indicate that we should stop eating.

Levels of our natural fullness hormones fall quickly after being released, which is why we often feel hungry again shortly after eating.

By contrast, the new obesity medication creates a more enduring signal to the brain, and therefore prolongs the feeling of fullness.

This means a person taking this medication will feel full for longer and be satisfied with a smaller amount of food. There is also evidence that these medications can reduce cravings.
The weight loss medication also slows the transit of food in your digestive system, which in turn allows the food you have eaten to stay in your stomach for longer. This contributes to a reduction in hunger levels and increased levels of satiety.

This reduced desire to eat food, when supported with a healthy eating regime and regular exercise, can help you lose weight over time.

While it is still recommended to make nutritional and lifestyle changes to support weight loss, medication such as Mounjaro, Wegovy, Ozempic and Saxenda can help to make the process more sustainable.

HOW CAN A TEAM OF HEALTHCARE PROFESSIONALS ENSURE THE SAFE USE OF WEIGHT LOSS MEDICATION?

Weight loss medications such as Mounjaro, Wegovy, Ozempic and Saxenda are safe and effective drugs for most adults that need them.

However, these medications are not the right treatment for everybody looking to address their weight, and there are important reasons why some people should not take these drugs.

In addition, while these medications are usually well tolerated, some people experience gastro-intestinal side effects such as nausea.

For these reasons, it is important that use of these medications is overseen by a specialist Multidisciplinary Team (MDT).

At WeightMatters, we assess your medical history to ensure that these medications will be a safe and effective choice for you.

After an initial metabolic assessment with our medical team, you may wish to meet with our clinical dietitians for regular monitoring and support.

The team will guide and coach you to maximise the benefits from using weight loss medication safely, in combination with dietary and lifestyle changes.

In some cases, we may also recommend that you work with one of our weight loss therapists, especially if you are aware of limiting beliefs or self-sabotage behaviour that may derail your best efforts.

WEGOVY ®

Wegovy ® is an injectable prescription medicine, and when combined with healthy changes to dietary intake and increased physical exercise, can support people who are obese in losing weight, and also reduce the risk of cardiovascular events such as heart attack or stroke.

What is Wegovy?

Wegovy is a brand name for semaglutide, a prescription medication used for chronic weight management in adults who are obese or overweight with at least one weight-related condition such as high blood pressure, type 2 diabetes, or high cholesterol.

How does Wegovy help with weight loss?

Wegovy works by mimicking a hormone called GLP-1 (glucagon-like peptide-1) that targets areas of the brain involved in regulating appetite and food intake. This helps to reduce hunger, increase feelings of fullness, and consequently decrease calorie intake.

How effective is Wegovy for weight loss?

Clinical trials have shown that individuals taking Wegovy can lose an average of 15% of their body weight over 68 weeks, compared to about 2.4% in those taking a placebo.

Who can take Wegovy?

A4: Wegovy is approved for adults with a body mass index (BMI) of 30 or higher (obese), or a BMI of 27 or higher (overweight) with at least one weight-related medical condition.

What are the common side effects of Wegovy?

Common side effects include nausea, diarrhoea, vomiting, constipation, abdominal pain, headache, fatigue, dyspepsia, and dizziness. These side effects are generally mild to moderate and tend to decrease over time.

How is Wegovy administered?

Wegovy is administered as a once-weekly subcutaneous injection. It comes in a pre-filled pen that you or a caregiver can easily use at home.

Is Wegovy approved for managing type 2 diabetes?

While Wegovy is primarily approved for weight management, it contains the same active ingredient (semaglutide) as Ozempic, which is specifically approved for managing type 2 diabetes. Therefore, it can indirectly benefit individuals with type 2 diabetes by promoting weight loss, which improves glycemic control.

How does weight loss from Wegovy impact type 2 diabetes?

Weight loss can significantly improve blood sugar levels, reduce the need for diabetes medications, and lower the risk of diabetes-related complications.

Who should not take Wegovy?

Wegovy is contraindicated in the following situations:

  • Personal or Family History of Medullary Thyroid Carcinoma (MTC): Patients with a personal or family history of MTC should not use Wegovy.
  • Multiple Endocrine Neoplasia syndrome type 2 (MEN 2): Patients with this genetic condition should avoid Wegovy.
  • Hypersensitivity: Patients with known hypersensitivity to semaglutide or any of the product components should not take Wegovy.
  • Pregnancy: Wegovy is not recommended for use during pregnancy due to potential harm to the fetus. Women of childbearing potential should use effective contraception during treatment.
  • Breastfeeding: It is not known if Wegovy is excreted in human milk. A decision should be made whether to discontinue nursing or discontinue the drug, considering the importance of the drug to the mother.

Are there any other health conditions that require caution when using Wegovy?

Yes, caution should be exercised in patients with:

  • Pancreatitis: Wegovy may increase the risk of pancreatitis.
  • Kidney Impairment: Patients with renal impairment should use Wegovy with caution, as it can cause dehydration and worsen kidney function.
  • Gastrointestinal Disease: Patients with severe gastrointestinal disease, particularly gastroparesis, should use Wegovy with caution due to its gastrointestinal side effects.

MOUNJARO ®

Mounjaro is a medication designed to help you achieve your weight loss goals when combined with a healthy diet and exercise. It works by targeting multiple hormones that regulate appetite and glucose levels, making it a powerful tool in your weight management journey.

What is Mounjaro?

Mounjaro (tirzepatide) is a medication that mimics the effects of both GLP-1 and GIP (glucose-dependent insulinotropic polypeptide) to enhance weight loss and improve glycemic control in individuals with type 2 diabetes.

How does Mounjaro aid in weight loss?

Mounjaro helps with weight loss by reducing appetite and calorie intake while also increasing energy expenditure. It acts on receptors in the brain that control hunger and satiety.

How effective is Mounjaro for weight loss?

Clinical studies have shown that Mounjaro can lead to significant weight loss, with patients losing an average of 15-20% of their body weight over 72 weeks, depending on the dosage.

Who is eligible to use Mounjaro for weight management?

Mounjaro is typically prescribed to individuals with type 2 diabetes, but its weight loss benefits can be considered for overweight or obese individuals, even those without diabetes, under a healthcare provider’s guidance.

What are the side effects of Mounjaro?

Common side effects include nausea, vomiting, diarrhoea, decreased appetite, abdominal pain, and constipation. These are usually mild to moderate and tend to diminish over time.

How is Mounjaro administered?

Mounjaro is administered as a once-weekly subcutaneous injection using a pre-filled pen, similar to other GLP-1 receptor agonists.

How does Mounjaro help manage type 2 diabetes?

Mounjaro lowers blood sugar levels by increasing insulin secretion, decreasing glucagon secretion, and slowing gastric emptying, which helps in better blood glucose control.

How effective is Mounjaro for type 2 diabetes management?

Clinical trials have demonstrated that Mounjaro can significantly lower HbA1c levels, with many patients achieving target HbA1c levels of less than 7%.

Can Mounjaro replace other diabetes medications?

Depending on individual responses and healthcare provider recommendations, Mounjaro may reduce the need for other diabetes medications. However, it should not be used without consulting a healthcare professional.

Are there any serious risks associated with Mounjaro?

A10: While serious side effects are rare, they can include pancreatitis, gallbladder disease, and hypoglycemia when used with insulin or sulfonylureas. Always consult with a healthcare provider for a full risk assessment.

Who should not take Mounjaro?

Mounjaro is contraindicated in the following situations:

  • Personal or Family History of Medullary Thyroid Carcinoma (MTC): Similar to Wegovy, patients with a personal or family history of MTC should not use Mounjaro.
  • Multiple Endocrine Neoplasia syndrome type 2 (MEN 2): Patients with this condition should avoid Mounjaro.
  • Hypersensitivity: Patients with known hypersensitivity to tirzepatide or any of the product components should not take Mounjaro.
  • Pregnancy: Mounjaro is not recommended for use during pregnancy due to potential harm to the foetus. Women of childbearing potential should use effective contraception during treatment.
  • Breastfeeding: It is not known if tirzepatide is excreted in human milk. A decision should be made whether to discontinue nursing or discontinue the drug, considering the importance of the drug to the mother.

Are there any other health conditions that require caution when using Mounjaro?

Yes, caution should be exercised in patients with:

  • Pancreatitis: Mounjaro may increase the risk of pancreatitis.
  • Kidney Impairment: Patients with renal impairment should use Mounjaro with caution, as it can cause dehydration and worsen kidney function.
  • Gastrointestinal Disease: Patients with severe gastrointestinal disease, particularly gastroparesis, should use Mounjaro with caution due to its gastrointestinal side effects.
