MEET PAULA, EXERCISE PSYCHOLOGIST

Dr Paula Watson describes how exercise anxiety can become a barrier to finding regular movement in your life.

Imran – Do You Resonate With His Story?

Imran, 42, found himself at a crossroads when he was diagnosed with Type 2 diabetes. This new diagnosis compounded his existing health concerns: high blood pressure, high cholesterol, and lifelong obesity. The recent referral to a cardiologist for chest pains served as a stark wake-up call.

His demanding job as a middle manager, with its stressful and ever-changing shift work, left him little time for self-care. Married with three young children, Imran’s deteriorating health became a growing concern for his family.

Imran’s once active lifestyle had dwindled due to his weight and health issues. He struggled with a poor diet, exacerbated by cultural restrictions around food, and found exercise unenjoyable and increasingly difficult.

The diabetes diagnosis and cardiologist referral were a turning point. Determined to see his children grow up and have families of their own, Imran resolved to reclaim his health and prioritise longevity.

Imran’s search for a solution led him to seek a clinic that could offer holistic care. He needed a team that understood the complexities of his situation and could provide integrated support alongside his wider medical treatment.

For Imran, finding a team that could offer psychological, emotional, and nutritional guidance was crucial to achieving lasting wellbeing.

Vicious Cycle

The Vicious Cycle of Chronic Illness & Obesity

Chronic illness and disease can trigger a cascade of physical changes in the body, which can, in turn, fuel disordered eating, poor mental health, and weight gain. This creates a complex and interconnected web of challenges that can be difficult to untangle.

Physical Changes & Disordered Eating:

  • Hormonal Imbalances: Chronic diseases often disrupt hormonal balance, impacting hunger cues, metabolism, and cravings. This can lead to overeating, restrictive eating, or other disordered eating behaviors.
  • Medication Side Effects: Certain medications used to manage chronic illnesses can cause weight gain, nausea, or changes in taste and appetite, contributing to unhealthy eating patterns.
  • Fatigue & Reduced Mobility: Chronic illness often leads to fatigue and reduced physical activity, making it challenging to maintain a healthy weight and increasing the risk of weight gain.
  • Digestive Issues: Conditions like irritable bowel syndrome (IBS) or inflammatory bowel disease (IBD) can cause digestive problems, leading to food avoidance, nutrient deficiencies, and unhealthy eating habits.

Mental and Emotional Impact:

  • Stress & Anxiety: The stress of managing a chronic illness can trigger emotional eating and unhealthy coping mechanisms, leading to weight gain and further health complications.
  • Depression: Feelings of hopelessness, sadness, and low self-esteem associated with chronic illness can contribute to emotional eating, social isolation, and a lack of motivation to engage in healthy behaviors.
  • Body Image Issues: Changes in body shape or function due to illness can negatively impact body image and self-esteem, potentially leading to disordered eating and unhealthy weight control behaviors.

The Obesity Link

Obesity itself is a chronic disease that can exacerbate existing health conditions and increase the risk of developing new ones. The physical and emotional challenges associated with obesity can further contribute to disordered eating, poor mental health, and a downward spiral of health decline.

    • Identify nutrient deficiencies, food sensitivities, and potential drug-nutrient interactions.
    • Create a tailored meal plan that addresses specific needs, manages symptoms, and supports overall health goals.
    • Incorporate foods that help manage inflammation, regulate blood sugar, and promote gut health.
    • Provide education on healthy cooking methods, portion control, and label reading.
  1. Micronutrient Optimisation:
    • Assess micronutrient status through blood tests and dietary analysis.
    • Identify and address any deficiencies through targeted supplementation and dietary adjustments.
    • Prioritise nutrients that support immune function, energy metabolism, and overall health.
    • Monitor and adjust supplementation as needed based on ongoing assessments and individual responses.
  1. Gut Health Restoration:
    • Assess gut health through symptom analysis, dietary evaluation, and potentially specialised testing (e.g. microbiome analysis).
    • Recommend dietary and lifestyle modifications to promote a healthy gut microbiome, such as increasing fibre intake, incorporating probiotic-rich foods, and managing stress.
    • Consider the use of prebiotics, probiotics, or other gut-supporting supplements as needed.
    • Address any underlying digestive issues, such as food sensitivities or intolerances, through dietary adjustments and targeted interventions.
  1. Behavior Change Support:
    • Utilise motivational interviewing to identify and address barriers to healthy eating.
    • Help individuals develop realistic goals and create sustainable behavior change strategies.
    • Provide ongoing support and accountability through regular check-ins or online resources.
    • Address emotional eating and disordered eating patterns through mindful eating practices and therapeutic interventions.

Advocate for the individual’s nutritional needs and provide education to other healthcare providers about the role of nutrition in chronic disease management.

Coordination With Medical Team:

  • Collaborate with the wider WeightMatters MDT, or the individual’s external healthcare providers, to ensure a coordinated and comprehensive approach to care.
  • Share relevant information about the individual’s nutritional status, dietary intake, and progress towards goals.
  • Adjust the nutrition plan as needed based on medical recommendations, medication changes, or other health developments.

Therapy

As mentioned previously, living with chronic illness and diseases, and its complex interplay with weight, can create a cascade of psychological, emotional and social challenges for an individual. Here are some interventions our therapy team may use to support someone:

1. Cognitive Behavioral Therapy (CBT):

  • Identify and challenge negative thought patterns and self-defeating behaviors that contribute to unhealthy eating habits, lack of exercise, and poor self-care.
  • Develop coping skills to manage stress, anxiety, and depression related to chronic illness and obesity.
  • Set realistic goals and create action plans to improve overall health and well-being.

2. Mindfulness-Based Practices:

  • Teach mindfulness techniques, such as meditation and deep breathing, to help individuals become more aware of their thoughts, feelings, and bodily sensations.
  • Cultivate self-compassion and acceptance of their body and health condition.
  • Develop healthier relationships with food and exercise by promoting mindful eating and movement practices.

3. Acceptance & Commitment Therapy (ACT):

  • Help individuals accept the realities of their chronic illness and obesity without judgment or avoidance.
  • Identify personal values and goals that align with a healthier lifestyle.
  • Commit to taking action towards those goals, even in the face of challenges and setbacks.

4. Body Image Therapy:

  • Address negative body image and self-esteem issues that often accompany chronic illness and weight struggles.
  • Develop a more positive and accepting relationship with their body.
  • Challenge societal beauty standards and promote body diversity and self-love.

5. Graded Exercise Exposure

  • Conduct a thorough assessment of the individual’s physical capabilities, limitations, and any underlying health conditions.
  • Develop a personalised exercise plan that starts at a manageable level and gradually increases in intensity and duration over time.
  • Incorporate a variety of exercises that improve cardiovascular fitness, strength, flexibility, and balance, while accommodating any physical limitations.
  • Monitor the individual’s response to exercise and adjust the plan as needed to ensure safety and effectiveness.
  • Provide education on the benefits of exercise for managing chronic illness, disease, and obesity.
  • Address any fears or anxieties related to exercise and promote a positive and enjoyable experience.
  • Help individuals overcome barriers to physical activity and develop sustainable exercise habits.

We know that therapy requires tailoring to the individual’s specific needs and preferences, considering the nature of their chronic illness, their mental and emotional state, and their personal goals.
